Here is a handy notebook to record important field information, including: name and address, SAMPLE information, vital signs, times, etc. Each notebook has forty sets of forms; one original/master (white) and a carbonless copy (yellow). There are two handy sizes for your choosing. The back cover provides reference information, including "The Rule of Nines" chart, pupil gage, and places to keep agency contact information.

A great way to record that critical field information. The original can be turned over to the EMTs upon their arrival or medical authorities when appropriate, and you retain a permanent copy for your records.
